Pleurisy

Accumulation of serous fluid between visceral and parietal pleura is called pl effusion or pleurisy

Causes

-severe mal nutrition
2. Exudative
-TB
-malignancy
-pneumonia,
-ca bronchus
-pulm infarction
-rh.arthritis
-pancreatitis
-SLE
-pleural mesothelioma
-meigs synd.
-drup induced

SYMPTOMS
-evening rise of temp
-dry cough
-breathlessness
-chest pain

SIGNS
-diminished movements
-fullness of intercostal spaces
-apical impulse not visualised
-trachea shifted to opposite side
-vocal fremitus n resonance diminished
-stony dull note on percussion.

Investigations
-complete blood count
-chest x ray PA view
-aspiration of pleural fluid for gram staining,ZN stain,culture
-sputum exam
-mantoux test
-FNAC
-pleural biopsy
-bronchoscopy thoracoscopy
-ultrasono,CT,MRI.

Treatment
-rest in bed with good nutritious diet
-NSAID to relieve pain
-aspiratiön of pleural fluid
-chest physiotherapy
-antiTB chemotherapy
-Rx of malignancy
